A 64-year-old businesswoman presents to the emergency room of our hospital in the evening, due to acute pain in her lower abdomen. Asked for her medical history, she reports not having any pre-existing conditions or past sugeries, she takes no regular medication, and she has never smoked.

Cystoscopy results do not reveal pathlogic findings, ureter stents (pigtail catheters) are places bi-laterally. CT-scan thorax/abdomen/pelvis: bilateral hydronephrosis, left kidney << mute >>, i.e. without significant fuction; multiple pathologically enlarged lymph noddes in the retroperitoneal region, suspicion of right sided peritoneal and pleural carcinomatosis and of multipe bone metastases (figure 1)

CT-guided reroperitoneal biopsy carried out.
